What Is Pocket Reduction?
Pocket reduction (also known as periodontal pocket therapy or flap surgery) is a surgical periodontal treatment used when gum disease has progressed and deep pockets have formed between the teeth and gums.
These pockets trap bacteria, plaque, and tartar, making them difficult to clean effectively and allowing infection to persist.
The goal of pocket reduction is to gently access these areas, remove diseased tissue and bacteria, and reduce the depth of the pockets so that the gums can reattach more securely to the teeth.
At Signature Dental Aesthetics, we take a conservative, tissue-preserving approach wherever possible, focusing on restoring gum health while maintaining comfort and long-term stability.

What Does Pocket Reduction Treat?
This treatment is recommended for:
- Advanced gum disease (periodontitis)
- Deep periodontal pockets around teeth
- Persistent gum inflammation and bleeding
- Bone loss around teeth due to infection
- Loose teeth caused by periodontal disease
- Difficulty maintaining oral hygiene in deep pockets
How The Treatment Works
The SDA Process
Periodontal Assessment & Digital Imaging
We begin with a detailed evaluation of your gum health, including measurement of pocket depths and assessment of bone support. Digital X-rays and Cone Beam CT (CBCT) imaging may be used to assess the extent of bone loss and guide precise treatment planning.
Comfort Preparation & Treatment Planning
The area is fully numbed using local anesthesia to ensure a comfortable experience. We carefully plan access to affected areas, prioritizing tissue preservation and minimizing disruption to healthy gum structures.
Deep Cleaning & Pocket Reduction Procedure
The gums are gently lifted to allow access to the root surfaces. Piezo surgical instruments and fine periodontal tools are used to remove bacteria, tartar, and infected tissue from deep pockets. The root surfaces are thoroughly cleaned and smoothed to encourage healthier reattachment of the gums.
Gum Repositioning & Healing Phase
Once the area is thoroughly cleaned, the gums are carefully repositioned and sutured to reduce pocket depth. As healing progresses, the gums adapt to their new position, helping create a healthier and more maintainable oral environment.
Many of our surgical treatments can be performed under moderate conscious sedation to maximize comfort and reduce treatment anxiety. Sedation is administered by a board-certified anesthesiologist, allowing patients to feel relaxed throughout more complex procedures.

Frequently asked questions
The procedure is performed under local anesthesia, and most patients experience only mild discomfort during recovery.
Initial healing typically takes 1-2 weeks, with continued gum stabilisation over the following months.
If oral hygiene is not maintained, gum disease may return. Regular hygiene visits and good home care are essential to prevent recurrence.
Gums may appear healthier and more tightly adapted to the teeth after healing.
Early-stage gum disease is often managed with non-surgical treatment, but pocket reduction is recommended when disease has progressed.
Many of our surgical and more complex procedures can be carried out under moderate conscious sedation, helping you feel calm and comfortable throughout treatment. Sedation is provided by a board-certified anesthesiologist, allowing us to offer a high level of care for patients who may feel anxious or are undergoing more involved procedures.
